FREQUENTLY ASKED QUESTIONS
ABOUT THE LEO PHARMA® CONNECT
CO‑PAY SAVINGS CARD

The LEO Pharma® CONNECT Co‑Pay Savings Card materials say that I need to have commercial insurance to be eligible. What does commercial insurance mean?

Commercial insurance—also known as private insurance—refers to health insurance that is not run by or paid for (even in part) by any state or the federal government. Most people get commercial health insurance through their employer, or by purchasing an individual plan on their own.

I have a commercial insurance plan, and I’m also enrolled in Medicare. Can I use the LEO Pharma® CONNECT Co-Pay Savings Card?

No. Federal law prohibits co-pay savings programs like those offered by LEO Pharma® CONNECT to be used by people with any form of government insurance—even if the person also has a commercial health insurance plan. You cannot be enrolled in any state or federally funded program, including but not limited to Medicare (including Medicare Part D), Medicaid, Medigap, VA, DOD, TRICARE, or CHIP.

What prescriptions can I use my LEO Pharma® CONNECT Co-Pay Savings Card for?

Your LEO Pharma® CONNECT Co-Pay Savings Card can be used when you fill a prescription for Enstilar® (calcipotriene and betamethasone dipropionate) Foam 0.005%/0.064%, Taclonex® (calcipotriene and betamethasone dipropionate) Topical Suspension 0.005%/0.064%, Picato® (ingenol mebutate) gel 0.015%, 0.05%. Can I use my LEO Pharma® CONNECT Co-Pay Savings Card at any pharmacy?

The LEO Pharma® CONNECT Co-Pay Savings Card is valid at all participating retail pharmacies in the United States. This includes most large pharmacy chains, as well as many independent pharmacies.

Can I use the LEO Pharma® CONNECT Co-Pay Savings Card for refills of my covered LEO Pharma® medication?

Yes, refills of covered LEO Pharma® medication are included, and each refill counts toward the annual maximum of 12 fills—or up to $6,000—per year.

I have prescriptions for more than 1 covered LEO Pharma® medication. Can I use one LEO Pharma® CONNECT Co-Pay Savings Card for both/all of them?

Yes, you can use the same LEO Pharma® CONNECT Co-Pay Savings Card each time you fill (or refill) a prescription for a covered LEO Pharma® medication—up to a total of 12 fills per year, or a maximum benefit of $6,000.

How do I know if my commercial insurance covers my LEO Pharma® medication?

When you fill your prescription at the pharmacy, show your activated LEO Pharma® CONNECT Co-Pay Savings Card to your pharmacist. The pharmacist will tell you if your insurance covers your medication, and how much your out-of-pocket cost will be.

If you have questions about your co-pay amount, your eligibility for the LEO Pharma® CONNECT Co-Pay Savings Card, or if your co-pay is more than $125 after applying the LEO Pharma® CONNECT Co-Pay Savings Card, call 1-855-820-9627 and select option 4.

Can I use the LEO Pharma® CONNECT Co-Pay Savings Card if I have a health savings account (HSA) or a flexible savings account (FSA)?

There are no restrictions on using the LEO Pharma® CONNECT Co-Pay Savings Card for people with HSAs or FSAs. Patients should check with their plan administrator to determine eligibility for paying co-pay amounts using their HSA or FSA as individual plans may vary.

I gave the pharmacist my LEO Pharma® CONNECT Co-Pay Savings Card, but the pharmacist says my co-pay is more than $20. Why is that?

Your actual out-of-pocket costs depend on your insurance plan. One reason why your out-of-pocket cost may be more than $20, even after the co-pay card is applied, is that you may not have met your plan’s annual deductible. A deductible is an amount of money that you need to pay each year for your prescription medications before your insurance will start to pay a portion of the costs.

If, after applying your LEO Pharma® Co-Pay Savings Card, your out-of-pocket amount is more than $125 (based on your insurance plan), you may be eligible for a new savings option that offers a convenient way to pay no more than $125 (for each prescription of Enstilar® 60 g, Picato® 0.015% or 0.05%, or Taclonex® Topical Suspension 60 g) and have your medication delivered to you. To find out if you qualify, call 1-855-820-9627, and select option 4.

My doctor says there’s a way I can get co-pay savings and have my LEO Pharma® medication delivered to me. How do I sign up for that?

Under the LEO Pharma® CONNECT Direct option, your doctor sends your prescription directly to a participating pharmacy fulfillment provider (you will be able to select a pharmacy from those participating in our program). You will receive a call from a LEO Pharma® CONNECT patient support specialist to confirm your insurance coverage and determine the lowest co-pay available. Then, your prescription will be delivered to you—in most cases within 24 hours, based on when your information is received. (Some locations may require delivery times up to 5 days.)

As with the LEO Pharma® CONNECT Co-Pay Savings Card, most eligible patients with commercial insurance will pay as little as $20 for each prescription of covered LEO Pharma® medication. When using the LEO Pharma® CONNECT Direct Option, the $50 or $125 co-pay amounts do not contribute toward an insurance plan deductible.
