You cannot be enrolled in any state or federally funded program, including but not limited to Medicare (including Medicare Part D), Medicaid, Medigap, VA, DOD, TRICARE, or CHIP.

Yes, refills of covered LEO Pharma® medication are included, and each refill counts toward the annual maximum of 12 fills—or up to $6,000—per year.
Yes, you can use the same LEO Pharma® CONNECT Co-Pay Savings Card each time you fill (or refill) a prescription for a covered LEO Pharma® medication—up to a total of 12 fills per year, or a maximum benefit of $6,000.
When you fill your prescription at the pharmacy, show your activated LEO Pharma® CONNECT Co-Pay Savings Card to your pharmacist. The pharmacist will tell you if your insurance covers your medication, and how much your out-of-pocket cost will be.

There are no restrictions on using the LEO Pharma® CONNECT Co-Pay Savings Card for people with HSAs or FSAs. Patients should check with their plan administrator to determine eligibility for paying co-pay amounts using their HSA or FSA as individual plans may vary.
Your actual out-of-pocket costs depend on your insurance plan. One reason why your out-of-pocket cost may be more than $20, even after the co-pay card is applied, is that you may not have met your plan’s annual deductible. A deductible is an amount of money that you need to pay each year for your prescription medications before your insurance will start to pay a portion of the costs.
